Bakgrunn: Sykepleiere har en viktig funksjon som veiledere for sykepleierstudenter i klinisk praksis. Flere studier viser at pasientrelaterte oppgaver og organisatoriske rammer konkurrerer med veilederrollen, noe som oppleves som krevende. Likevel opplever de fleste sykepleiere at veilederrollen er givende. Det er funnet få studier som viser hva sykepleiere fokuserer på i veilederrollen. Hensikten med denne studien er å utforske hva sykepleiere anser som viktig i rollen som veiledere av sykepleiestudenter i klinisk praksis. Metode: Studien har en hermeneutisk tilnærming, basert på åtte intervju med sykepleiere som veileder studenter i praksis i spesialisthelsetjenesten. De transkriberte tekstene blir analysert ved hjelp av en hermeneutisk metode basert på Gadamers filosofi. Funn: Analysen resulterer i fire tema hvorav to har to undertema. Temaene omhandler forutsetningene for en god læringsprosess; kartlegging av studentens læringsforutsetninger; ansvarsforhold i veiledningsprosessen; refleksjon og logg som verktøy i veiledningsprosessen. Konklusjon: Studien viser at veilederne legger stor vekt på å avdekke og korrigere holdninger som de anser som uforenlig med å være en god sykepleier, og de anser motivasjon og trygge relasjoner som forutsetninger for en god læringsprosess. Veiledere legger vekt på kunnskapsformidling og påtar seg mesteparten av ansvaret for studentens læring. Refleksjon og logg brukes som verktøy i veiledningsprosessen.

Background and purpose: Nurses have an important function as mentors for nursing students in clinical practice. Several studies show that patient-related tasks and organizational frameworks compete with the supervisor role, which is perceived as demanding. Nevertheless, most nurses find the supervisor role to be rewarding. There are few studies showing what nurses focus on in the role of supervisor. The purpose of this study is to explore what nurses consider important in their role as supervisors of nursing students in clinical practice. Method: The study has a hermeneutical approach, based on eight interviews with nurses who supervise students in specialized health care services. The transcribed texts are analysed using a hermeneutic method based on Gadamer’s philosophy. Findings: The analysis resulted in four themes and four subthemes: Preconditions for a good learning process; mapping the learning assumptions of the students; assigning responsibilities in the supervision process; reflection and logs as tools in supervision. Conclusion: The study shows that motivation and good relationships between the supervisors and the students were important when preparing for a good learning process. The supervisors put great emphasis on students’ attitudes. They saw themselves as communicators of knowledge and took most of the responsibility for successful supervision. Reflection and logs were used to optimize the students’ professional development.

Bakgrunn: Foreldre deler opplevelser av å ha syke barn i sosiale medier. Dette er lite beskrevet og så vidt vi vet, den første studien som utforsker norske barnesykepleieres erfaring med at foreldre eksponerer syke barn i sosiale medier. Hensikt: Utforske og beskrive barnesykepleieres synspunkter og erfaringer med foreldre som omtaler egne barn innlagt på sykehus, via bilder og tekst i sosiale medier. Videre å få innsikt i om, og eventuelt på hvilken måte, eksponeringen påvirker pasient/foreldre- barnesykepleierforholdet. Metode: To fokusgruppeintervjuer med til sammen 11 barnesykepleiere på to sykehus ble utført. Analysen ble basert på Giorgis prinsipper om kategorisering og meningsbærende enheter. Resultater: Studien avdekket fem hovedresultater: Barnesykepleierne erfarte at foreldre eksponerte barna for å få støtte fra omverdenen. Barnesykepleierne hadde erfaringer med og synspunkter om at foreldres eksponering kunne være skadelig for barnet. Barnesykepleierne hadde ulike synspunkter om foreldres eksponering av premature og nyfødte. Barnesykepleierne kunne vegre seg for å ha ansvar for pasienter det ble blogget om. Barnesykepleierne leste foreldres blogger som omhandlet syke barn av ulike årsaker. Konklusjon: Resultatene viser at barn/foreldre-barnesykepleierforholdet kan bli påvirket når foreldre eksponerer barnet i sosiale medier.

Background: Parents share their experiences of having their hospitalized child exposed in social media. Little knowledge exists about this, and as far as we know this is the first study to explore Norwegian pediatric nurses experience of parents’ exposure of ill children in social media. Purpose: Explore and describe pediatric nurses’ experiences and views of parents exposing their sick children in social media through pictures and stories. And to gain insight into whether and in what way, the exposure affects the patient/parent- and the pediatric nurse relationship. Method: Two focus group interviews were conducted with totally 11 pediatric nurses from two hospitals. The analysis was based on Giorgis principles of categorization and meaningful entities. Results: The study revealed 5 main findings: Pediatric nurses experience; parents expose their children in order to get support from the outside world. Pediatric nurses experience and opinion; parents' exposure could be detrimental for the child. Pediatric nurses’ opinion of parents´ exposure of premature and newborn children in social media, differed. Pediatric nurses could be reluctant to take responsibility for patients whose parents have a blog. Pediatric nurses follow parents’ blogs for various reasons. Conclusion: This study suggests that the relationship between patient/parent and pediatric nurses is affected when parents expose their ill child in social media.

The study aimed to gain knowledge about concept-based change work by health professionals in new roles as resource persons implementing the Joy of Life Nursing Home concept. There is a need for cultural change in nursing homes to contribute to meaningful lives for the residents. The concept “Joy of Life Nursing Home” has been created to ensure quality standards in nursing homes. This study is about the experiences of members of a project team as change agents working to implement the Joy of Life concept in one nursing home that is endeavoring to become certified. A qualitative design was used. The sample included a certification project group from one nursing home; it consisted of five health professionals designated as resource persons, two department managers, and a process supervisor. Data were collected through focus group interviews twice during the certification process. Transcribed data were analyzed using thematic content analysis and interpretative phenomenological analysis of the focus group data. The analysis resulted in four themes: benefits and challenges of systematization work; being a resource person for colleagues; role prerequisites of the resource group; and Joy of Life as a concept and way of working. The study was on formative evaluation research, analyzing an implementation resource group’s experiences of the strengths and weaknesses of the program implementation process. In meeting positive attitudes about the concept, the resource persons succeeded. However, in the face of negative attitudes, they had limited skills to meet resistance. Limited supervision in the resistance the resource persons encountered limited the implementation process as a learning process. Role clarification and cooperation in JLNH made the work easier, and Joy of Life as a concept became an important motivation in the implementation work.

This article explores the possibilities for nurse teachers’ professional development as perceived by nurse teachers. An interpretative qualitative approach was employed and methodology triangulation was applied. Three main themes captured the factors that facilitate nurse teachers’ professional development—the organization, collegiality, and the qualities of the nurse teachers themselves—and these factors’ interplay is complex. Long-term improvement is more likely to come from sustained supportive efforts at individual education sites with teachers (including leaders) working collaboratively to respond to the specific needs of their students and the constraints of their context. The experiences gained in this study suggest taking an integrative approach to nurse teachers’ professional development, including and balancing influencing (demanded and enacted upon) factors at individual, team, and organizational levels.

Koordinering af kræftforløb har stort fokus i Danmark. Der har igennem de sidste 10 år været stort fokus på at lette de organisatoriske overgange i kræftforløbene. Studiets formål var at få indsigt i kvinders oplevelser af organisatoriske overgange i forbindelse med at være i behandling for brystkræft. Der blev udført 12 kvalitative, semistrukturerede interview med kvinder midt i deres behandlingsforløb. Analysen bestod af en systematisk tekstkondensering for at identificere meningsbærende enheder, der blev kodet til to temaer: 1) kvindernes oplevelse af manglende kontrol af selve behandlingsforløbet – hvem holder styr på mig? og 2) kvindernes oplevelse af manglende sammenhæng i overgangene mellem afdelingerne. Undersøgelsen viser, at et øget fokus på at lette overgangene i behandlingsforløbet kan være med til at øge kvindernes oplevelse af kontrol, og dermed mulighederne for at klare de udfordringer, der er i et kræftbehandlingsforløb. Her spiller sygeplejersker som koordinator en meget vigtig rolle i at skabe overblik og kontinuitet, som er grundelementerne i at klare de udfordringer, der er i at gå fra at være syg til at blive rask og fremadrettet mestre livet. Disse sygeplejersker skal have en tæt relation til kvinderne udover kun sikre, at forløbstiderne overholdes.

Cancer case management has attracted major focus in Denmark. During the past 10 years, there has been focus on relieving the organizational transitions in the cancer processes. The purpose of this study was to gain insight into women’s experience of the organizational transitions in connection with being treated for breast cancer. Twelve qualitative, semi structured interviews were conducted with women half way through their treatment. The analysis consisted of a systematic text condensation to identify meaningful units coded to two themes: 1) the women’s experience of lack to little control with the treatment process – who is keeping track of me? and 2) the women’s experience of lack of cohesion in the transitions between departments. Studies show, that increasing awareness on easing the challenges in the treatment process, plays a factor in increasing the women’s experience of control, and thereby the possibility of developing coping mechanisms. Nurses play an integral role as a case manager in creating overview and continuity, which are the key elements in manage the challenge from being sick to becoming well and able to cope life. The case manager not only ensures that the treatment process is as scheduled but must build a close relationship with the women.

Psykiatriloven hjemler en række forskellige tvangsformer når særlige kriterier er opfyldt, heriblandt f.eks. tvangsindlæggelse, tvangsfiksering, indgivelse af beroligende medicin med tvang og tvangsmedicinering. Endskønt det således i lovgivningen anerkendes, at psykiatritvang nogle gange er nødvendig, er der megen opmærksomhed på at nedbringe brugen. Sundhedspersoner er i princippet ansvarlige for at tvang (kun) anvendes, hvor den er lovlig. I dette essay søges det indkredset, hvordan man som ansat i psykiatrien står ansvarsmæssigt i forbindelse med brug af tvang. Udgangspunktet tages i lovgivningen og case beskrivelser. En væsentlig nedbringelse af tvangsindgreb i psykiatrien synes i princippet at kunne opnås ved undgåelse af ulovlig tvang. Sygeplejefagligt personale er i psykiatrien individuelt ansvarlig i henhold til autorisationslovens krav om omhu og samvittighedsfuldhed (§ 17) også når det gælder tvangsanvendelse. I praksis omhandler psykiatriske patientklagesager dog først og fremmest klager over selve tvangsanvendelsens lovlighed, medens sundhedspersoner til sammenligning med det somatiske område kun sjældent stilles disciplinærretligt til ansvar. I de situationer, hvor der eksplicit gives udtryk for kritik, har det ofte sammenhæng med dokumentationsmangler snarere end den individuelle sundhedsfaglige ageren. Således understreges vigtigheden af en løbende og relevant journalføring også i relation til tvangsudøvelse, men der antydes tillige et relativt tomrum i sundhedspersoners disciplinæransvar.

As is the case in other countries, Danish legislation permits psychiatric coercion in some special situations and health professionals are responsible for only instigating coercive measures when satisfying legal requirements. In this essay, this responsibility is discussed with reference to formal law and case law. It is highligthed that by preventing unlawful utilization, coercion in principle could be considerably reduced. When using coercive measures, nursing staff and other authorized health persons are professionally responsible according to the authorization act’s claim for diligence (Para 17). Nonetheless, contrary to other parts of the healthcare system, patient complaints about application of coercive measures usually are handled as cases about lawfulness only and rarely are managed as malpractice cases about individual health professional responsibility. This suggests a relative gray area. Besides, a great proportion of cases concern documentation issues again drawing attention to the importance of thorough medical records keeping.
