Hensikten med evalueringsstudien er å identifisere medvirkende faktorer til opprettholdelsen av et bærekraftig generasjonsoverskridende program. I et lokalsamfunn i Norge har det i forlengelsen av et prosjekt i 2006 eksistert et fast månedlig program for eldre og førskolebarn. Våren 2011 ble programmet evaluert gjennom fokusgruppeintervju med ansatte og frivillige som hadde vært med i en ansvarsgruppe siden oppstart. I evalueringen ble det vektlagt fokus på helsefremmende faktorer og organisatoriske forhold knyttet til programmet. Det ble gjort induktiv innholdsanalyse av materialet. Faktorer som fremkom som betydningsfulle for opprettelsen av det generasjonsoverskridende programmet var opplevelse av nytte og mening ved å bidra, samarbeid, støtte og tilrettelegging samt en balanse mellom forutsigbarhet og fleksibilitet i programmet. Disse faktorene samsvarer med helsefremmende perspektiver. I artikkelen ses funnene opp mot komponentene i Antonovskys teori om Opplevelse av sammenheng, hvor særlig meningsfullhet som motivasjonskomponent viste seg å samsvare med informantenes bidrag i programmet.

Evaluation of an intergenerational program within a theoretical framework of health promotion – Which factors have contributed to the endurance of the program?

The aim of the present study is to identify contributing factors to the endurance of an intervention concerning intergenerational relations. As a result of a project conducted in 2006 there was established an intergenerational program in a local community in Norway for elders and pre-school children. In 2011 this program was evaluated through a focus group interview with employees and volunteers that had contributed in the program since the start. Primary focus of the interview was health promotion characteristics and practical organizing of the intergenerational program. Inductive qualitative content analyses were performed. The most prominent factors considering endurance of the intervention was the experience of use and meaningfulness, cooperation, support and facilitation in addition to a balance between predictability and flexibility in the program. These factors correspond to health promotion perspectives. In this article they are being related to the components of the theory of Antonovsky, Sense of Coherence, and especially meaningfulness as the motivational component correlated well to the informants’ contribution in the program.

Sammanfattning

Syftet med studien var att beskriva grundutbildade sjuksköterskors upplevelse av att börja arbeta på en barnavdelning. En kvalitativ intervjustudie har utförts med ändamålsenligt utvalda informanter. Semistrukturerade intervjuer genomfördes med åtta grundutbildade sjuksköterskor. Det insamlade materialet analyserades med kvalitativ innehållsanalys. Resultatet visade att informanterna upplevde att det var ett för kort avsnitt om pediatrik i grundutbildningen till sjuksköterska. Erhållen kunskap räckte inte till för professionellt arbetet med barn. De flesta ansåg att det var genom verkliga situationer i yrkeslivet som de lärde sig nödvändig kunskap. Vidare visade det att endast ett fåtal var nöjda med introduktionen. Att gå med många olika handledare var en viktig faktor för upplevt missnöje med introduktionen. Bra stöd erhölls från medarbetare, däremot upplevdes att stödet från organisationen var bristfälligt. Slutsats: Dagens grundutbildning till sjuksköterska innefattar inte tillräckligt med pediatrik för att förbereda den studerande för professionellt arbete med barn och ungdomar. Insatser av individen och arbetsplatsen krävs för att öka kunskapsnivå hos den grundutbildade sjuksköterskan. Kontinuitet och stöd under introduktion är viktigt för att de nya sjuksköterskorna ska känna sig trygga i sitt arbete.

Basic trained nurses experience to start working on a children’s ward

The aim of the study was to describe nurses trained to a basic level experience of starting work on a children's ward. A qualitative study has been conducted with a purposively drawn sample of informants. Semi-structured interviews were conducted with eight basic trained nurses. Data were analyzed using qualitative content analysis. The results showed that respondents felt that the pediatrics course is not preparing for the professional work with children. Most felt that it was through real situations in the workplace that they gained necessary knowledge. Furthermore, it was found that only a few were satisfied with the induction. Spending time with many different supervisors was an important factor for experiencing dissatisfaction with the induction. Good support was received from employees; however the support of the organization was poor. Conclusion: Today's undergraduate education in nursing does not include enough pediatrics to prepare students for professional work with children and adolescents. Actions at individual and workplace levels are needed to increase knowledge of the basic trained nurse. Continuity and support during the induction is important for newly employed nurses to feel competent in their work.

Participatory Action Research in clinical nursing practice in a medical ward – challenges and barriers

Background: Action research with a participatory approach (PAR) was used as research design in a medical ward but stopped midway because of lack of active actor participation in the actions. Aim: To describe challenges and barriers influencing lack of participation. Setting: A medical hospital ward, Denmark. Participants were healthcare staff. Methods: Field observations, interviews, logbook. Data were analysed using content analysis methods. Findings: Multiple factors influenced lack of actor participation. The causes were complex and included: organizational framework, significance/meaning, actor roles, responsibility. Conclusion: Before using PAR it is crucial to investigate if the organization and the participants at all levels are suited and agree to participate actively. The findings indicate, that to carry out PAR in a busy medical ward, it is necessary to evaluate whether the necessary framework and conditions are present not only prior to but during the entire project process. Implications. The study shows that PAR is not always suitable as research approach in a busy hospital ward. Furthermore, the study outlines methodological questions in relation to use of PAR.

Respecting as a basic teamwork process in the operating theatre

Background: Studies show that working as a team in the operating theatre can be a challenge, and that surgical teams are not so cohesive as might be expected. Methods: A qualitative approach with exploratory design was used, inspired by grounded theory (GT) as a methodology. The data are from semi–structured, open questions, interviews with eight female theatre nurses from four Norwegian operational units. Results: The study shows that the most important factor in the social process between theatre nurses co-operating with other team members, is respecting. This includes interactions where: the attitudes expressed make team members feel valued; team members show an understanding for each other; team members communicate constructively. The leader’s role is to encourage the right attitudes, focusing on respect for one another. Conclusion: The study shows that cooperation is an inter-human process, which requires the presence of multiple factors in the relations between professionals. Organizational factors also play a decisive role in enhancing the quality of inter-disciplinary work. Strengthening teamwork can be a complex task in a multidisciplinary surgical team where conflicting professional identities exist. Here, organisational factors play a crucial role in improving the quality of interdisciplinary collaboration.

Introduksjon: Verdighet er et grunnleggende behov for mennesker og en kjerneverdi i sykepleien. Å eie en indre opplevelse av verdighet er av stor betydning, spesielt for eldre pasienter. Hensikt og mål: Studiens formål er å belyse hvordan eldre pasienters behov for å opprettholde egen opplevelse av verdighet kommer til uttrykk under sykehusopphold. Økt bevissthet rundt tema vil gjøre det lettere for helsepersonell og gjenkjenne pasienters uttrykk og respondere på dette. Metode: Studien er kvalitativ og har et utforskende/beskrivende design. Hermeneutisk tilnærming er valgt. Innsamling av data skjedde ved hjelp av halvstrukturerte intervju av eldre pasienter med lårhalsbrudd. Systematisk tekstkondensering var veiledende i analysen av intervjuene. Funn: Respondentenes behov for å opprettholde egen opplevelse av verdighet kom til uttrykk gjennom respondentenes presentasjon av verdier som hadde stor betydning for dem å leve etter. Dette var verdiene; selvstendighet, ansvars – og omsorgsroller og ordentlighet. Konklusjon: Å få uttrykke disse verdiene, få bekreftelse på dem og hjelp til å etterleve dem hjalp respondentene til å opprettholde verdighet. Respondentenes uttrykk blir da en forespørsel som helsepersonell må motta og handle etter.

«Take care of my dignity»

Introduction: Dignity is a basic need for human beings and a core value in nursing. It is of vital importance to possess an inner sense of dignity, especially for elderly patients. Purpose and aim: The purpose of this study is to shed light on how elderly patients express their need to maintain their own dignity during their stay in hospital. Increased awareness of this topic will make it easier for healthcare professionals to recognize these expressions and respond to them. Method: This is a qualitative study with a explorative/descriptive design, using a hermeneutic approach. Data were collected using a semi-structured interview of elderly patients with hip fracture. The interviews were analyzed using systematic text condensation. Results: The respondents’ need to maintain their own sense of dignity was expressed via the mention of values that the patients felt it important to live by. These values were: independence, roles of responsibility and caring, and orderliness. Conclusions: Being allowed to express these values, having them reaffirmed and being helped to live by them, helped the respondents to maintain their dignity. Thus, the respondents’ expressions become requests that healthcare professionals must respond and act according to.

With an aging general population and a concurrent increase in the prevalence of dementia, nursing students are increasingly exposed to this group of patients during their clinical placements and after graduation. A sound dementia-related knowledge base among nursing students is important in providing high-quality dementia care. The present study assessed Alzheimer’s disease knowledge in nursing students. The Alzheimer’s Disease Knowledge Scale (ADKS) was utilized to measure the level of dementia knowledge. A total of 230 undergraduate nursing students in their final years of study, but prior to graduation, participated in this study. The study identified a moderate Alzheimer’s disease knowledge base (mean score 24.52 out of 30) among nursing students. The results revealed that the students also had some misconceptions pertaining to Alzheimer’s disease. In addition, significant differences in knowledge were evident between educational institutions in ADKS content domain ‘caregiving’ (p=0.001). These results suggest that the current dementia curriculum should be evaluated in an attempt to improve the Alzheimer’s disease knowledge base of nursing students.
