Læbe-ganespalte i et pædodontisk perspektiv

Læbe-ganespalte (LGS) er den hyppigst forekommende ansigtsmisdannelse med en prævalens i Danmark på ca. 2 tilfælde pr 1.000 nyfødte. LGS klassificeres i undergrupper i forhold til deres oprindelse og placering (eks. isoleret læbespalte); ca. 1/3 af tilfældene er isoleret læbespalte, 1/3 er isoleret ganespalte, og 1/3 er kombineret læbe- og ganespalte. Danmark har Verdens ældste centraliserede og standardiserede gratis behandlingssystem for LGS-patienter, hvilket betyder høj og ensartet behandlingskvalitet samt unikke forskningsmuligheder inden for området. I det følgende beskrives og diskuteres de typiske ansigtstræk, den tidlige kæbevækst, samt påvirkningen af dentitionen, og almene tandsundheds overvejelser hos børnepatienten med LGS.

Cleft lip and palate (CLP) is the most common congenital malformation of the face (occurrence in Denmark 2:1000 newborns). The Danish Health Care has a longstanding tradition (since the 1930’s) for support of standardized and centralized, and free treatment for patients born with a cleft malformation. This is organized by the two cleft lip and palate centers in Denmark and the related university hospitals, and include all treatment related to the cleft condition such as e.g. plastic surgery, orthodontics, and speech training, whereas «ordinary» dental treatment is related to the patients «ordinary» dentist (in municipal pediatric or adult private dental care). The access of free care, a unique high standard of treatment, as well as a very homogenous population have resulted in exceptional possibilities for research in CLP in Denmark. This paper summarizes the treatment system, the most common cleft types, as well as the early development of the face and the plausible related disturbances in the dentition and challenges in oral health in children with cleft lip and palate.
